Requirements
When you go through online booking or contact us to make an appointment we will need a few things from you including your name, address, and phone number.
Once we know who you are, where you are, and how to contact you we can schedule your first appointment and send you intake forms that you can complete prior to us arriving. Completing these forms prior to your appointment helps to streamline your first visit. If you are unable to complete the forms beforehand we will have you complete them when we arrive.
To ensure our safety we have several policies which include patients being responsible for having available parking, clearing walkways of snow/ice, and proper sanding/salting. Please clear a space within a suitable living area in your home for us to set up our treatment tables (which are approximately 2 feet by 6 feet) and enough room for us to work around it. Appointments will not be conducted in personal bedrooms.
